These accomplishments helpedbring orthodontics and orthodontists to a reasonablelevel of sophistication both diagnostically and techni-cally.Metallurgical innovations of orthodontic wires,bonding of brackets, new bracket designs, and newimaging techniques have made orthodontics more effi-cient,as well as more patient friendly.Although such technical advances have had a sig-nificantimpact on the practice of orthodontics, the cli-nicianstill must develop the same time-honored skillsto be able to properly diagnose and plan treatment foreach patient. In other words, the clinician still must ac-quirecertain basic tools that enable him or her to syn-thesizerelevant information.

The clinician must thenapply this information to the individual patient in asystematic and consistent way.This book is a modest attempt to provide such infor-mationin a logical format that helps students of ortho-donticsboth understand and apply the basic concepts ofdiagnosis and treatment. Without such a basic founda-tion,diagnosis and treatment planning for a malocclu-sionbecomes a difficult and misguided exercise forwhich prospective patients will bear the consequences.The contributors to this text have worked hard topresent their ideas in a format that is both clear as wellas scientific. Reading the works of all of these authorshas been an education as well as a revelation of thedepth of their understanding and their abilities. To eachone, I owe a debt of gratitude!The text is organized into six sections:Section I: Growth and Development.

This section de-scribesthe overall physical changes that occur betweenthe embryonic stage and adulthood. Special emphasisis given to the normal and abnormal development ofthe facial structures as well as to the development ofthe dentition and the occlusal relationships from theprimary to the permanent dentitions.Section II: Diagnosis.

The systematic evaluation ofthe various dentofacial relationships is essential forthe proper diagnosis of malocclusions. This sectiondiscusses the details of how each parameter needs tobe evaluated for the clinician to be able to differenti-atebetween relatively simple and more complexproblems involving the dentition and skeletal andsoft tissue structures.Section III: Appliances. The principles that govern theconstruction and the use of fixed appliances are pro-vided.The emphasis is on biomechanical conceptsrather than on details of various techniques.Section IV. Treatment and Treatment Considerations.The treatment approaches to different malocclusionsthat the clinician faces in the primary, mixed, and per-manentdentitions are discussed. The complexities ofthe orthodontic treatment are outlined as they relate tospecific malocclusions.Section V: Other Aspects Related to Treatment. In ad-ditionto the proper diagnosis of the malocclusion andthe choice of the appliance to be used, other parame-tersexist that need to be considered by the clinician,the patient, and the parents to successfully accomplishthe desired treatment objectives.Section VI: Orthodontics and Adjunct Treatment.

Anumber of areas are considered to be an intricate partof the overall evaluation and management of the or-thodonticpatient. Some of these problems need eithera special consideration from the orthodontist or theexpertise and intervention of other specialists to pro-videthe patient with the optimal treatment for a givenmalocclusion.This text is not meant to provide a definitive ap-proachto all types of malocclusion.

On the otherhand, I hope that it provides sufficient information toenable the clinician to properly diagnose the com-plexitiesof a given problem while also giving anoverview of various treatment approaches to some ofthese problems.Samir E.
